Fitness Coaches are staff items in FIFA 21 Ultimate Team which increase the rate of fitness cards when applied to player. This means, by having Fitness Coaches in your FUT Club, you help your players to get fit faster when a fitness card is applied to them.
FUT 21 Fitness coaches come in three Bronze, Silver and Gold qualities and have different bonus rates. The bonus rate of a FIT Coach item is the incremental percentage rate added to a fitness card when that fitness card is applied to an injured player. Here is the list of Fitness Coach card qualities:

Bonus Rates
The total of your Fitness Coach cards rates will be added as additional bonus to an applying fitness card until the rate hits 50%. This means, if the total of your bonus rates goes over 50%, it is only consider 50%. For example, if you are having 11 Rare Gold Fitness Coaches in your club, the total of these cards will be 55%, but you will only get 50% as for the bonus rate.
How to Apply a Fitness Coach to a Fitness Card?
No action is required to engage a Fitness Coach for using its bonus rate on a fitness card. All you need to do is to keep your Fitness Coaches in your FUT Club. The bonus rate will be automatically added to all healing cards you have where applicable.
Where to Find Fitness Coaches?
Fitness card items can be found in the packs and can be received as rewards. They also can be purchased from the Transfer Market, by searching under Staff tab and choosing the Staff Type as Fitness Coach.

No. The people names and faces used for the Fitness coach cards in FIFA Ultimate Team are in fact taken from real people who are working at EA. These people are actually EA developers and designer who mostly were engaged with FIFA game development.
